Going Forward

• To Establish links with Care homes, and one trained

advocate to build relationships and offer outreach

services

• To get the rest of the GPs on board to utilise their

dementia registers

• To offer outreach in the local hospitals

• Continue to listen to those with dementia and their

carers to help improve, initiate and praise services
