• Often health and social care services work separately, yet from the
perspective of the people who use these services, they need to work
together.
• Autumn 2017 we commenced reviews of 20 local authority areas about
how services work together to support older people aged 65 and older.
Some of these reports are now available. Four key themes are being
explored:
1 System leadership
2 Maintaining people’s well being in their usual place of residence
3 Care and support when people experience a crisis
4 Step down, return to usual residence and/or admission to a new
place of residence
• Individual local reports are being published as the reviews are
completed and a national report will be published in the summer of
2018
